In addition to filling out the Registration Form, please also print the Emergency Info & Consent-to-Treat Form below and submit the completed form to UUSG via mail or in person. (For privacy reasons, we don't offer the option to fill out this form online.)

If your child is in 6th-12th grade and will be participating in any overnights or field trips, please note that we also require a photocopy of the student's photo ID and a photocopy of the insurance card they're covered under (if applicable). You can bring photocopies to the church or we can copy the items here.
